Select the on axis screw, duplicate it and rotate the dupe -120 degrees.

Select the off axis screw and rotate it -109 degrees.

To create the 0.375 diameter socket head recessed cap bolt used on the crank enlarge the horizontal dimension of the original on-axis screw to 0.459 inches (the lock should be closed).

Since this sits in a counter bored hole very little of the height will be visible. Use the node edit tool to drag the bottom nodes up about as shown. Group the result and rotate 120 degrees.
